Apple Health

Medicaid is Apple Health.

Will you qualify for Washington Apple Health?

If your income is below the following, you may be eligible for the new Medicaid program. FPL means "Federal Poverty Level." These numbers are estimates and may change.

Medicaid members still enrolled under the Healthy Options or Children's Health Insurance Program will need to renew their coverage and enroll in Apple Health. We are providing a number of options to ensure a smooth transition between programs.

Community HealthFirst - Medicare Advantage

Our Community HealthFirst™ Medicare Advantage Plans are designed to provide access to quality health care providers, benefits, information, and services that will help you stay in the best health possible.

Our affordable Medicare Advantage HMO plans give members valuable extended coverage and services:

Understanding and trusting your health care options is critical.

Community HealthEssentials Plus is a health insurance coverage plan offered through the Washington State Health Benefits Exchange.

There are three different Community HealthEssentials coverage levels available in 26 counties: Gold, Silver, and Bronze.

Community HealthEssentials Plus is offered by Community Health Plan of Washington and administered by First Choice Health.

Community Health Plan of Washington has partnered with First Choice Health to provide the administrative services for our Community HealthEssentials Plus private health insurance plans. This includes Customer Service, Claims Processing, Utilization and Case Management, and access to the First Choice Health PPO Network.

First Choice Health has the largest independent PPO network in the state. Members enjoy access to more than 25,000 providers, including 100+ Community Health Center clinics, 500 affiliated clinics, and more than 100 hospitals.

Please note: Community HealthEssentials Plus is the only Community Health Plan of Washington plan that uses the First Choice Health PPO Network.

Will I be able to keep seeing my doctor after I become a member?

We contract with more than 2,500 primary health care providers and more than 14,300 medical specialists. To find out if your current provider is affiliated with Community Health Plan of Washington, search our online directory. You can also call Customer Service at 1-800-440-1561 (TTY Relay: Dial 711), 8 a.m. to 5 p.m., Monday through Friday.

How do I know if I should see a doctor?

Our free Nurse Advice Line is available 24 hours a day, 7 days a week. If you need help deciding on the care you need, you can call 1-866-418-1002 (TTY: 1-866-418-1006) for assistance.

How do I make an appointment?

To make an appointment, call your clinic or doctor's office. If you need help locating the phone number, please use our provider directory online.

When you call to make an appointment:

Please tell them if you are a new member.
Or ask for an interpreter if you need one.

Will I get an ID card from Community Health Plan of Washington?

When you enroll in Community Health Plan of Washington, you will be sent an ID card. Each person in your family who signs up with Community Health Plan of Washington will get their own ID card.

You will need to show the Community Health Plan of Washington ID card and your state-issued Services card each time you get medical care. That includes medical visits, specialist visits, mental health visits, hospital visits, and pharmacy prescriptions.

What if I want a second opinion about my health care?

You have the right to get a second opinion. Tell your primary doctor that's what you want. They will refer you to a different provider within the Community Health Plan of Washington network with similar credentials, or you can choose one from our provider list.
